- Please take time to read my terms of business page. The terms page of my website is printer-friendly. These terms form part of my contract with you. A contract agreement is appended to this document.
- Once I have confirmed your booking with you, please print out a copy of the terms of business and sign a copy and post it to me. If this is not done then the terms stated in the current version of the terms of business document nevertheless apply.
- Please check that the practical items (for example, computer log-in, visiting drug bag) as set out in the terms of business document have been attended to where necessary.
